Zantac Dosing for GERD or Erosive Esophagitis
For treating erosive esophagitis or GERD in children or babies, the usual dose of Zantac is typically 5 mg to 10 mg per kg (a little less than 2.5 mg to 5 mg per pound) total per day. This is usually split up into two doses per day.
 

Zantac for Babies: Summary

Zantac is approved for use in babies and children and comes in two convenient dosage forms. However, you should always talk to your baby's healthcare provider before giving any medication to your baby. Do not give over-the-counter Zantac to babies.
